1. A 60 year old female receiving chemotherapy receives a platelet transfusion. She soon develops fever and rigor and is found to be significantly hypotensive.
2. A 30 year old male receives a bone marrow transplant. Over the next few days the patient develops severe diarrhoea, abdominal pain and vomiting as well as a rash that initially started on the palm of the hands, but soon became widespread.
3. A 20 year old female with known IgA deficiency develops dyspnoea, stridor, severe hypotension and tachycardia following a blood transfusion.
4. A 30 year old male involved in a road traffic accident (RTA), becomes restless and develops fever and chills immediately following blood transfusion. The patient later develops pink urine. A patient involved in a similar incident was brought in at the same time.
5. A 42 year old woman develops shortness of breath and hypoxia following blood transfusion. On x-ray she is shown to have pulmonary oedema. She has no previous history of cardiac disease
